Case 119

 A sixty three-year-old male had a periodic health care examination once a year after endoscopic subcutaneous dissection (ESD) for early gastric cancer.He had smoked 40 cigarettes/day until forties. He has a habit of drinking two bottles of beer almost every day. He has felt a little fatigue these days. He has hepatitis B virus. His farther had mitral valve insufficiency and his mother experienced to receive valve replacement.

 Laboratory test revealed brain natrium peptide 300 pg/dL (<40). Chest radiograph showed suspicious plural effusion.

 He agreed to receive enhanced cardiac CT using contrast medium.
