A Case of the Week

Case 127

A fifteen-year-old boy presented in our hospital to receive pelvic MRI with an invitation letter from the local orthopedic clinic. The letter said that he suffered from pain of left sacroiliac joint to left hip joint. The prescribed pain killers were not effective for his pain. Then, further examination using MRI was necessary. He was a right hander pitcher of little league baseball. The pain increased when throwing a ball in a pitching form. Pelvic MRI he received was shown in Figs 1-3.

What is your imaging diagnosis based on pelvic MRI?

1. Arthritis

2. Osteomyelitis

3. Stress fracture

4. Fibrous dysplasia
