A seventy-year-old female presented in our hospital for dyspnea. She had various diseases such as diabetes mellitus, hypertension, rheumatoid arthritis, rheumatoid lung and medication-related osteonecrosis of the jaw, thrombophlebitis of the lower extremities and atrial fibrillation. She received home oxygen therapy and steroid therapy for a long term. Her oxygen pressure was 99 mm Hg under oxygen inhalation of 2L/min. Laboratory test revealed white blood cells (WBC) 4870, lymphocytes 15.4%, CRP 22.87 mg/dL β-D glucan 43.6 pg/mL on the admission day and WBC 5790, lymphocytes 14.0 % six days after when she underwent chest CT.
Based on clinical findings and chest CT, what is your diagnosis?
