Case 174

 A seventy eight-year-old female presented in our hospital for being hard to breathe and moisture cough. She was suffering from rheumatoid arthritis (RA) for years. She was diagnosed as RA-related interstitial pneumonia and received home oxygen therapy (HOT) of 1L/min, maintaining PO2 89-90%. She was feeling symptoms like common cold for a few days. She was given steroid and erythrosine from a local clinic. Laboratory test revealed WBC 23390/mm3, neutrocytes 85.2%, CRP 2.06 mg/dL, RF 67 IU/mL, Procalcitonin 0.14ng/mL, BNP 64 pg/mL.
 She received chest CT for further examination (Figs. 1-3).

What is your diagnosis ?

1. Usual interstitial pneumonia

2. Non-specific interstitial pneumonia

3. Diffuse pan-bronchiolitis

4. Follicular bronchiolitis

5. Sarcoidosis
