Case 192

 A forty two-year-old male was transported to our hospital with repeated vomiting two hours after massive intake of water in a toilet. He had a severe mental retardation. Previously, he did the same behavior at times but this time the water volume was extraordinary. Laboratory test revealed Na 126 mEq/L, K 3.5 mEq/L, CRP 0.02 mg/dL, White blood cell 2770/mm3, platelets 183000/mm3, procalcitonin 18.57ng/mL. His mother and he agreed to take abdomen CT.

What is your diagnosis on abdomen CT?

1. Gastric ulcer perforation

2. Duodenal ulcer perforation

3. Transverse colon perforation

4. Colon cancer

5. Sigmoid colon diverticulitis
