Case 212

 Case 1
An eighty-year-old male presented in our hospital for persistent lumbago for approximately one week. At first, he suffered from left sided lumbago and then, followed by right-sided lumbago, eventually bilateral lumbago. He felt his bodyweight heavier. It was very hard for him to rise up and to bend or extend his knees. Motor function of lower extremities: 5/5. Sensory perception of lower extremities: no disorder. Laboratory test revealed CRP 10.36 mg/dL, white blood cells 9210/mm3, monocyte 11.3%, RF 43 IU (< 15). He received lumbar MRI (Figs 1. 2).

What is your diagnosis for Case 1 ?

1. Multifidus muscle injury

2. Supraspinous muscle injury

3. Interspinous muscle injury

4. Facet joint injury

5. Peri spinous fat edema


 Case 2
A fifty six-year-old female presented in our hospital for lumbago. She fell down three days ago and she did again, yesterday. Neither palsy nor abnormal skin perception of lower extremities was found. She received lumbar MRI (Figs 3-5).

What is your diagnosis for Case 2?

1. Multifidus muscle injury

2. Supraspinous ligament injury

3. Interspinous ligament injury

4. Facet joint injury

5. Sclerosing panniculitis
