Case 30

 A fifty one-year-old male went to a local clinic for chest pain where he was indicated to have plural fluids. He was introduced to our hospital for further investigation. His condition was extremely poor, nearly in a shock state with shallow respiration. He smoked more than 40 cigarettes and drunk a lot everyday. His pH, SpO2 and PCO2 in room was 7.32, 79.1% and 56.4%, respectively. His respiration sound of right lung was hardly listened. Laboratory test revealed CRP 31.36 mg/dL, White blood cells 20640/mm3, Neutrophils 90.2%, Blood sugar 193 mg/dL and BUN 54.4 mg/dL. He received chest radiograph and CT for further examination.

What is the appropriate imaging diagnosis ?

a. Lobar pneumonia and pleuritis

b. Pneumothorax with pleural fluids

c. Pulmonary abscess

d. Pyothorax

e. Pulmonary abscess + pyothorax
