Case 61

 A sixty nine-year-old male first visited the dermatology department of our hospital for suffering pain, redness and swelling at the underside of the right forefoot. He mentioned that he had felt painful only when weight pressure was given to his big toe for the previous 6 months but, in these days, he felt more painful even at rest. He was a handball player. He wanted to know what happened in his big toe. Then, he was introduced to our orthopedist who found the painful underside of his first big toe corresponded to the 1st matacarpophalangeal (MP) joint. Laboratory test revealed no abnormal values except total cholesterol 244 mg/dL, CRP 0.12 mg/dL and eGFR 58.8 ml/min/1.73 m2.

 Because toe radiograph was found within normal limits, toe MRI was conducted for further investigation.
