Case 70

 Case 1. A sixty three-year-old female came to our hospital suffering from cough, malaise and general fatigue. Several weeks ago, she experienced dyspnea during music concert. Her house was old, dusty and moldy-smelling. Saturation PO2 lowered and fluctuated, 80 to 94 mmHg. Body temperature was 36.7 centigrade. Heart rate was 90/min. She was admitted to our hospital for further examination. Laboratory test revealed no abnormal data except LDH 361 mg/dL, and positive Trichosporin antibody test. She took Chest CT (Figure 1) for further examination.


 Case 2. A sixty three-year-old female presented in our hospital, suffering from dyspnea at rest for the past two weeks. Dyspnea was repeatedly recovered and worsened during the period. Last month, she experienced eosinophilic pneumonia with elevated Ig E. Laboratory test revealed CRP of 0.48 mg/dL, white blood cells of 6010/mm3, eosinophils of 6.7 %, and positive Trichosporin antibody test. She agreed to take Chest CT (Figure 2) for further examination.


What is the imaging diagnosis on chest CT of both patients?

1. Usual interstitial pneumonia

2. Nonspecific interstitial pneumonia

3. Cryptogenic organizing pneumonia

4. Acute hypersensitivity pneumonia

5. Acute respiratory distress syndrome
