Case 74

 A seventy three-year-old male presented for cough and persistent fever for a couple of weeks. He was given medicine every three months for diabetes mellitus by a local clinic whose physician introduced him to our hospital. Normal breath sound was audible in auscultation. Laboratory test revealed CRP 2.30 mg/dL, white blood cells 5980 mm3, eosinophils 21.4 %, lymphocytes 19%, ALP 578 (100-340)U/L, total bilirubin 1.25 mg/dL, blood sugar 163 mg/dL. He received chest CT for further investigation (Figs. 1, 2, 3).

What is imaging diagnosis based on chest CT ?

1. Idiopathic pulmonary fibrosis/ Usual interstitial pneumonia

2. Lymphocytic interstitial pneumonia

3. Langerhans histiocytosis

4. Collagen vascular disease associated interstitial pneumonia/ non-special interstitial pneumonia

5. Lymphangioleiomyomatosis
