A Case of the Week

Case 87

A seventy seven-year-old female experienced inarticular speech in that morning when her daughter called her. Thereafter, when going out, she fell down in front of her house at 11 AM. Her neighborhood helped her to take to our hospital at 11: 30 AM. Because she seemed to be unable to walk smoothly, our security guard advised her to use a wheel chair and guided to the washroom. Our security guard found her falling down inside the washroom and took her to emergency room. She had diabetes mellitus, hypertension and previously experienced brain infarction. She was given medicines of anticoagulants and antihypertention for preventing re-attack of brain infarction. Her medical findings revealed anisocoria, positive left-sided Barre’s sigh and slight left hemineglect. Other findings are Japan coma scale, I-1; blood pressure, 138/81 mmHg; blood sugar 165 mg/dL: SpO2 84%. She gave her consents to receive brain CT(Fig.1-3) and two days later, brain MRI(Fig.4-6) for further investigation.

Based on symptoms and findings on brain CT and MRI, what main artery is involved in our patient?

1. Tuberothalamic (polar) artery

2. Paramedian (thalamic perforating = Percheon) artery
