 A forty nine-year-old female was transported to our hospital directly from medical university hospital with a referring letter. It said that she got common cold 4 months ago, and ten days later, she experience progressive weakness of right upper extremity, and rectal and urinary retention. She accepted to receive brain MRI at that time (Figs 1 - 6).

What is your diagnosis on brain MRI?

1. Optic neuromyelitis

2. Multiple sclerosis

3. Neuro-sarcoidosis

4. Acute disseminated encephalomyelitis

5. Vitamin 12 deficiency
