Case 206

 An eighty five-year-old female presented in our hospital by ambulance for being unable to speak clearly from this morning. Blood pressure 181/76mmHg. Japan Coma Scale I-2, Glasgow Coma Scale E4/V4/M6.
 Neurological test; right Barre’s sign, positive; right Mingazzini test; positive, speech mild dysarthria; right hemiparesis: mild rt. facial palsy.
 Laboratory test revealed fibrinogen 578.6 mg/dL, WBC 5800/mm3, CRP 1.1 mg/dL, BUN 27.1 mg/dL, BNP 22.4 pg/mL.
 She received brain MRI for further examination (Figs 1, 2).
