A Case of the 10 days

 We present an eighty six-year-old male in our hospital for lumbago, fever and atrial fibrillation with a letter from a local clinician, saying he experienced atrial fibrillation three months ago and asking our cardiologist about application of direct oral anticoagulant. Laboratory test revealed blood cells 9420/mm3. BNP= 1013.2 pg/mL (< 18,5) ng/dL, PTINR 1.56 (0.8 – 1.2), CRP 7.53 mg/dL. He took CT for investigating fever origin (Figs 1 – 4).

What is your imaging diagnosis ?

1. Rupture of aortic aneurysm

2. Dissecting aneurysm

3. Inflammatory aortic aneurysm with ulceration

4. Retroperitoneal fibrosis
