A Case of the 10 days

Case 307

 An eighty-one-year-old female presented in our hospital with a letter from a local clinic, saying that she experienced cough triggering persistent sputum in throat and her chest radiograph showed left pleural effusion. Laboratory test revealed white blood cells 9150/mm3, platelet 474000/mm3, D dimer 9.7 microgram/mL. She took chest CT (Fig.1) and MRI (Fig. 2) for further investigation.

What causes hemi lateral pleural effusion in case of absence of pulmonary lesion ?
